1. That by completing the contact form, accessible at the bottom of these terms and conditions of use (the form), does not create any contractual obligation between Attorney Fernando Zambrana (Attorney) and my person (the parties), particularly in the nature of a Contract for the Provision of Professional Services, NOR DOES IT constitute an attorney-client relationship between the parties. 
2. That by completing the form I am merely requesting an appointment with the Attorney to discuss my case with the purpose of determining if we shall suscribe a Contract for the Provision of Professional Services, where, if agreed by the parties, the Attorney will assume my legal representation.
3. That the personal and private information that I will provide in the form is willfully and freely given.
4. That, furthermore, I understand that this information to be entered in the form will be provided with the purpose of facilitating the discussion of my case with the Attorney, where we shall determine whether he will assume my legal representation.
5. That, even though by submitting my personal and private information to the Attorney via this form does not create any contractual and professional obligation between the parties, such information is confidential and may not be disclosed to third parties without my express authorization.
